Benefit Options Wellness is a request-based program; classes and screenings are scheduled at worksites where an employee has requested them. Therefore, the Wellness program relies on State employees to bring Wellness events to their worksite.
Classes and screenings are provided by contracted professionals who travel throughout the State. Scheduling events is subject to vendor availability.
Requests should be submitted 5 to 6 weeks prior to the date being requested.

Wellness Library
Wellness Lending Library Operations Effective April 1, 2006 the Wellness program is no longer lending materials via interagency or US mail. Materials may be borrowed in person, on a self-service basis. The Benefit Options Wellness office is located at 100 N 15th Ave, Suite 103 Phoenix.
