SCREENING EVENTS

 

 

MINI HEALTH SCREENING

 This free screening includes:

Blood pressure; Body composition (weight and body fat percentage); Fasting cholesterol and full lipid panel*; Fasting blood sugar* *8-hour fasting is recommended for best result (water and medications are okay)

 Optional screenings available:

Osteoporosis Free for women 40 and older. $35 copay for women under age 40.

PSA (prostate-specific antigen) $5 for men 40 and older. $40 cost for men under age 40.

Typically, no appointments are required at scheduled events. Participants will be required to provide their Employee Identification Number (EIN).  Individual, confidential results will be mailed to each participant.  A copy of the test results can also be sent to the participant’s physician (participants need to provide physician contact information at the event if they want the report sent to their physician).  Mini-health screenings are typically scheduled early in the day as fasting is required for tests.

Cost: free, except for listed cost of optional screenings

Length: Screening takes approximately 10 – 15 minutes per person. Total event time will be based on the size of the worksite.

Participation: minimum of 20 participants required. Exception may be considered on a situational basis.

Please note: Mini Health Screening events are not scheduled during flu shot season (October thru December).

HOW TO REQUEST AND SCHEDULE WORKSITE EVENTS

 SKIN CANCER SCREENING

This screening includes:

Visual assessment (using “body mapping” technique) to check for suspicious lesions, along with education on how to prevent skin cancer and how to examine one’s body.

Assessment is performed by a nurse practitioner or physician assistant. This includes taking a personal and family history and identifying lifestyle factors.

Participants are not required to fully disrobe. Exam will be limited to exposed skin. Follow up recommendations are made based on finding.

Appointments are required at scheduled events.

Cost: free

Length: Screening takes approximately 10 minutes per person.

Participation: 3-hour (18-person) minimum per worksite is required. Screening events have a 6-hour (36 person) maximum.

HOW TO REQUEST AND SCHEDULE WORKSITE EVENTS

 MAMMOGRAPHY

Mammography (breast cancer) screening for women is provided by Mobile On-Site Mammography (M.O.M.). The M.O.M. mobile travels to perform mammography screening at worksites across Arizona (a minimum of 25 participants is required). Appointments are required at scheduled events.

M.O.M will directly bill insurance. Benefit Options health plan members do not pay a copay at these events. Participants will need to show their Benefit Options insurance card at the time of their appointment.

Confidential mammogram reports (results) will be sent to the participant and the physician they designate (participants need to provide physician contact information if they want the report sent to their physician).

Cost: Free for Benefit Options Health Plan members (non-members, see note below)

Length: Screening takes approximately 10 – 15 minutes.

Participation: minimum of 25 participants

Please note: Other insurance plan members may have a copay. Contact your insurance company’s member services department for more information.

M.O.M. events are not scheduled in November and December.
