The Mayo Clinic Health Risk Assessment (HRA) is a completely confidential tool that is useful to everyone! The HRA questionnaire gathers information to help you prioritize your health risks and motivates you to make healthy lifestyle changes. When you take the Mayo Clinic HRA, you may find out about health risks you did not realize you have.

Employees and spouses who complete the Mayo Clinic HRA through the website will be automatically entered to win one of five hundred $100 Premiere Choice Awards and you may be eligible for FREE lifestyle coaching
