Wellness Event Request
Benefit Options Wellness is a request-based program; classes and screenings are scheduled at worksites where an employee has requested them. Therefore, the Wellness program relies on State employees to bring Wellness events to their worksite. Classes and screenings are provided by contracted professionals who travel throughout the State. Scheduling events is subject to vendor availability. Requests should be submitted 5 to 6 weeks prior to the date being requested. Who is Eligible The Wellness program serves State employees, retirees and their families. All services and events are free to benefit-eligible employees as well as retirees and spouses on the Benefit Options health plan. Dependents are eligible for specific events as indicated on promotional materials (no worksite events). For all screening events, participants must know their (or their spouse’s) Employee Identification Number (EIN) and will be asked to show their Benefit Options insurance card or proof of State employment. Minimum Requirements There are a minimum number of participants required for all Wellness events at a worksite. Unless otherwise indicated, class minimum is 20 and screening minimum is 25. If your worksite is too small to meet the minimum, consider partnering with other nearby State offices or, contact Wellness to try to locate other Agencies in your area. Cost Wellness events are free to State Agencies and their offices. Some events have a participant co-pay or fee. Please read the class and screening descriptions for more details. What To Do Submit your event request: Event Request Promotion and Registration Wellness staff will provide you with promotional materials (generally via email, that you can use at your work site. The Wellness program asks worksites to do their own registration or take appointments for events that require it. Forms will be provided to assist you with this.
