Q. I have a disease/significant health issues and receive specialized care, how do I know these services will continue with the new plan?
A. We have developed a team of nurses and medical professionals to assist you during the transition to our new plan. Orchard Medical Consulting is an independent company devoted to this process. Please contact their office directly at (602) 542-5008 or 1-800-304-3687 so that we can arrange a medical specialist to personally contact you to assist with all of your needs and concerns.
Q. What benefits will be offered?
A. We will continue to offer medical, dental, vision, life insurance, short-term disability, and flexible spending account benefit options.
Q. What medical plans will be available?
A. Employees will be able to choose from an Exclusive Provider Organization (EPO) or a PPO (Preferred Provider Organization). The EPO provides services just like an HMO. Participants will be asked to select a primary care physician who will coordinate all of the participant’s care, however access to most specialists will not require a referral from their PCP. A PPO provides flexible access to care without a primary care physician. The EPO and PPO network(s) that are available vary by the employee’s county of residence.
   
Q. Is an HMO plan still going to be available?
A. Yes, but the name of the HMO plan will change. Self Insurance rules refer to the HMO model as an EPO or Exclusive Provider Organization.
   
Q. If I need to change doctors, how will the new doctor get my medical records?
A. You will need to give your new doctor permission to obtain your medical records. You can do this by filling out a form at the physician’s office or contacting your former doctor and providing them the information on your new doctor.
   
Q. My preferred doctor is not in any of the networks. What can I do?
A. If you provide us the name, address, and phone number of your physician, we will forward this information to the contracted networks to see if they can add your doctor to their networks. Networks continue to expand so please check the physician search of the network that you selected on a periodic basis.
   
Q. Will our medical benefits be cut with the new plan?
A. No. Medical benefits will be the same as or better than the medical benefits the State previously offered.
