Benefit Information

New State Employees: Newly hired and returning employees must enroll for benefits coverage within 31 days of their date of hire or reinstatement. If you are nearing the end of your 31-day enrollment period and are not able to enroll using the "Your Employee Services" (YES) website, contact your agency benefits liaison before the 31-day period has ended. The effective date for your benefits coverage will be the first pay period following receipt of a properly executed enrollment form and required supporting documentation (provided the request is received within thirty-one (31) days of the date of hire). Changes to Benefits: Except for plans with after-tax premiums, you may change your benefit elections during the year only when you experience a Qualified Life Event (QLE). If you have not experienced a QLE, you must wait until the next annual open enrollment period to make changes. Qualifying Life Events include but are not limited to:
- Changes in your marital status: marriage, divorce, legal separation, annulment, death of spouse;
- Changes in dependent status: birth, adoption, placement for adoption, death, or depedent eligibility due to age, marriage, and student status;
- Changes in employment status or work schedule that affect benefit eligibility for you, your spouse, and/or dependent;
- Changes in residence that result in different available plan options.

Eligible Depedents: Legal spouse or domestic partner as outlined below; Natural, adopted and/or stepchild unmarried and under age 19, or under 25 if a full-time student at an accredited educational institution; Minors under the age of 19 for whom the employee-member has court-ordered guardianship; Foster children under the age of 19; Children placed in the employee's home by court order pending adoption; Natural, adopted and/or stepchildren who were disabled prior to
age 19; or older child as outlined below.
Domestic Partners: must share residence with employee or retiree and have done so continuously for the past 12 months; not legally married to or separated from anyone else; not a close blood relative; at least 18 years old; meets certain financial interdependency tests. Older Child: is younger than 25 years; unmarried; was covered by an Arizona Department of Administration (ADOA) insurance plan while 18 years of age; resides in Arizona; and is a natural child, adopted, stepchild, or employee was a court-ordered guardian when the child was 18 years or younger.
